  1. Have the provider complete the TNC Provider Onboarding Form.
  2. Review the submission for missing credentialing information.
  3. Collect all supporting documents.
  4. Confirm names, dates, licenses, NPI, CAQH, and work history are consistent.
  5. Complete the final RISE readiness checklist.

  1. Complete onboarding information and document collection.
  2. Perform the packet quality check.
  3. Confirm CAQH/PECOS readiness when applicable.
  4. Submit through TNC’s current approved RISE credentialing workflow.
  5. Document that the packet was handed off.
  6. Remain available for provider signatures or additional documents.
After handoff, RISE owns assigned payer enrollment and status follow-up. The PM remains the local point person for missing provider information, signatures, location changes, and updated documents.

Confirm and communicate

Packet submission does not equal payer participation. Use confirmed status/effective-date information from the credentialing process before treating a provider as participating for a payer.
