Patient check-out is the final opportunity to ensure the patient understands their care plan, receives any required follow-up instructions, schedules future appointments, and leaves with a positive experience. An effective check-out process supports continuity of care and reduces missed follow-up.

Purpose

A standardized check-out process ensures patients receive appropriate follow-up care, understand their next steps, and leave the clinic with all necessary information, appointments, and resources.

Before a patient leaves, staff should verify provider orders have been addressed, future appointments are scheduled when appropriate, financial responsibilities have been discussed, and the patient understands the next steps in their care.

Important: Patients should never leave without addressing provider instructions, recommended follow-up appointments, or outstanding questions whenever possible.

Responsibilities

Front Office

Complete the patient check-out process, schedule follow-up appointments, provide visit documentation when applicable, and ensure provider orders are completed before the patient leaves.

Providers

Clearly communicate follow-up recommendations, referrals, testing, and return visit expectations.

Medical Assistants

Communicate provider orders requiring action before patient discharge and coordinate with Front Office when needed.

Billing

Assist with patient financial questions that cannot be resolved during check-out.

Standard Check-Out Workflow

Review Provider Orders

Review the provider's recommendations, including follow-up appointments, laboratory testing, imaging, referrals, prescriptions, or additional instructions before beginning check-out.

Schedule Follow-Up

Schedule recommended follow-up appointments using the Scheduling Appointments SOP and provider scheduling guidelines.

Coordinate Referrals & Orders

Ensure referrals, laboratory orders, imaging requests, or outside services are initiated according to clinic workflow.

Review Patient Responsibilities

Discuss any remaining financial responsibilities, provide receipts if applicable, and answer general checkout questions within your scope.

Provide Patient Instructions

Review follow-up recommendations, appointment details, and any provider instructions before the patient leaves the clinic.

Complete Check-Out

Update the appointment status in RXNT according to clinic workflow and verify all required checkout tasks have been completed.

Best Practice: Before the patient leaves, ask, "Is there anything else we can help you with today?" This provides one final opportunity to resolve questions and reinforces The Neighborhood Clinic's commitment to exceptional patient care.

Special Check-Out Situations

Some patients require additional coordination before leaving the clinic. Staff should ensure all provider recommendations have been addressed and appropriate follow-up arrangements have been initiated.

Laboratory Orders

Confirm laboratory orders have been entered, explain where testing will be completed, and answer general questions regarding the process.

Imaging Orders

Review imaging orders with the patient and explain how scheduling or authorization will occur according to clinic workflow.

Referrals

Ensure referral requests are initiated and explain the next steps for specialty care.

Behavioral Health Follow-Up

Schedule recommended follow-up appointments and provide any required resources or instructions before discharge.

Patient Education

Patients should leave the clinic with a clear understanding of their treatment plan, follow-up expectations, and any instructions provided by the healthcare team.

All checkout activities should be documented consistently within RXNT. Detailed navigation and screenshots are maintained in the RXNT Center.

Appointment Status

Update the patient's appointment status according to clinic workflow.

Follow-Up Appointment

Document or schedule follow-up visits before completing checkout whenever recommended.

Referral & Orders

Verify referrals, laboratory orders, imaging requests, and other provider orders have been initiated appropriately.

Patient Notes

Document objective checkout information that supports continuity of care.
