Patient Check-In
Patient check-in establishes the foundation for every visit by confirming the patient's identity, verifying demographic and insurance information, collecting required documentation, and preparing the patient for clinical care.
Purpose
A standardized check-in process improves patient safety, reduces registration errors, supports accurate billing, and ensures providers receive complete and current patient information before beginning the encounter.
Every patient should complete the established check-in workflow before being roomed by clinical staff. Front Office is responsible for ensuring all required information has been reviewed and updated.
Responsibilities
Front Office
Welcome the patient, complete registration, verify demographics and insurance, collect required forms, collect applicable patient balances, and complete check-in within RXNT.
Medical Assistants
Confirm the patient has been checked in before rooming and notify Front Office of any registration discrepancies identified during intake.
Billing
Assist with insurance or financial questions that cannot be resolved during registration.
Leadership
Monitor compliance with registration standards and provide coaching when necessary.
Standard Check-In Workflow
Welcome the Patient
Greet every patient professionally, acknowledge them promptly, and create a welcoming first impression.
Verify Patient Identity
Complete the Patient Identity Verification SOP before accessing or updating the patient's medical record.
Review Demographics
Verify the patient's address, phone number, email address, emergency contact, and preferred pharmacy. Update any changes immediately.
Verify Insurance
Confirm active insurance coverage and verify eligibility before completing the check-in process.
Collect Required Forms
Ensure all required registration forms, consents, and acknowledgements have been completed or updated when applicable.
Collect Patient Balance
Collect copays, outstanding balances, or self-pay amounts according to clinic policy before the patient is seen, unless otherwise approved.
Special Check-In Situations
Not every patient arrives with a standard registration scenario. Staff should follow clinic policy while maintaining excellent customer service and ensuring patient safety.
Late Arrivals
Follow clinic policy regarding late arrivals. Notify the provider or leadership when the patient's arrival may impact the schedule. Inform the patient that appointment availability may be affected.
Walk-In Patients
Follow the Walk-In Workflow SOP. Verify provider availability before completing registration and communicate estimated wait times when applicable.
Missing Insurance
Attempt to verify coverage using available resources. If insurance cannot be confirmed, discuss available self-pay options and notify the patient of any financial responsibility before services are rendered.
Incomplete Forms
Review all required forms before completing check-in. Assist patients with questions and ensure required signatures are obtained.
Complete Check-In in RXNT
Complete Registration Review
Verify that demographics, insurance information, required forms, and financial responsibilities have been reviewed before completing check-in.
Update Patient Status
Change the appointment status within RXNT to indicate the patient has arrived and completed the registration process.
Notify Clinical Staff
Ensure Medical Assistants are aware the patient is ready for rooming according to clinic workflow.
Customer Service Expectations
Acknowledge Every Patient
Patients should be greeted promptly upon arrival, even if another patient is currently being assisted.
Communicate Delays
If provider delays occur, proactively update patients and thank them for their patience.
Protect Privacy
Speak quietly when discussing personal or financial information and maintain HIPAA compliance at all times.
Create a Welcoming Environment

RXNT Documentation
Detailed screenshots and navigation instructions are maintained within the RXNT Center. Staff should document all registration activities consistently.
Arrival Status
Update the patient's appointment status appropriately within RXNT.
Registration Updates
Document demographic or insurance changes completed during check-in.
Financial Documentation
Document collected copays or patient payments according to clinic procedures.
Patient Notes
Record only objective information that assists clinical staff or future visits.
