Workflow Purpose

The RXNT patient portal and digital intake process allow patients to securely receive forms, submit demographic and clinical information, review documents, communicate with the clinic, and prepare for their appointment before arrival.

Staff are responsible for ensuring that forms are sent to the correct patient, portal access information is accurate, completion status is monitored, submitted forms are reviewed, and missing information is addressed before the patient is checked in.

Forms Must Match the Appointment

Do not send every available form to every patient. Select the appropriate forms based on whether the patient is new or established, the appointment type, age, service line, planned procedure, and provider requirements. Duplicate or irrelevant forms create confusion and may cause patients to overlook required documents.

Portal Privacy & Authorized Access

Portal access belongs to the patient unless a legally authorized representative, guardian, or approved proxy has documented access. Verify the caller’s identity and authority before discussing portal information, changing contact details, resending forms, or assisting with access. Never send an adult patient’s portal information to a spouse, parent, family member, or caregiver without appropriate authorization.

Confirm the correct chart, appointment, and contact information before sending any portal link or intake package.

  • Verify the patient’s full name and date of birth.

  • Verify the mobile number and email address directly with the patient.
  • Confirm the preferred language and communication method.
  • Review whether a guardian, proxy, or authorized representative is involved.

Select the Correct Forms

Choose forms based on the appointment rather than sending every available document.

  • Determine whether the patient is new or established.
  • Confirm the service line and appointment type.
  • Select age-appropriate and visit-specific screenings.

Intake Form Status Guidance

Sent

The form package was transmitted, but receipt or completion has not yet been confirmed.

Delivered

The message or link appears to have reached the verified destination. Completion is still required.

Opened

The patient accessed the form package but may not have entered or submitted all required information.

In Progress

The patient started the forms but required sections, signatures, or submission remain incomplete.

Completed

The patient submitted the package, but staff review and reconciliation are still required.

Needs Correction

Submitted information is missing, inconsistent, unsigned, or requires clarification.

Unable to Deliver

The link or message failed. Verify the contact information before resending.

Completed In Office

The patient completed required documents at the clinic using an approved process.

Portal Troubleshooting

Link Not Received

  • Verify the mobile number and email address.
  • Ask the patient to check spam or junk folders.
  • Confirm the message was sent successfully.

Link Will Not Open

  • Confirm the patient is using the most recent link.

Identity Verification Fails

  • Confirm the information entered matches the patient record.
  • Review spelling, date of birth, phone number, and email.

Forms Will Not Submit

Submitted Intake Review

Administrative Review

Verify demographics, contact information, insurance, emergency contact, preferred name, pharmacy, and required signatures.

Provider Review

Route responses requiring diagnosis, treatment decisions, urgent assessment, or provider interpretation.
