Patient Identification

Verify the correct patient before providing care, documenting, administering medication, collecting specimens, completing procedures, or releasing protected health information.

Purpose

Patient identification protects patients from errors and ensures all services, documentation, orders, medications, specimens, and records are connected to the correct individual.

Every team member is responsible for actively confirming identity. Staff must never assume a patient’s identity based on appearance, recognition, room location, appointment time, or information displayed on a schedule or computer screen.

Approved Patient Identifiers

Ask the patient to state the information whenever possible. Do not simply read the information aloud and ask the patient to agree.

Full Legal Name

Ask the patient to state their first and last name. Confirm the name matches the patient’s record.

Date of Birth

Ask the patient to state their complete date of birth and compare it with the information in RXNT.

Address

A current address may be used as an additional identifier when further verification is needed.

Phone Number

A phone number may be used as an additional identifier when identity remains unclear or records appear similar.

Room number, provider name, appointment time, and physical appearance are not acceptable patient identifiers.

Standard Identification Procedure

Complete these steps before beginning any patient-care activity.

Greet and Engage the Patient

Introduce yourself, explain what you are preparing to do, and speak directly to the patient whenever they are able to respond.

Ask for Two Identifiers

Ask the patient to state their full legal name and complete date of birth. Use an open-ended request rather than providing the answers.

Compare the Information

Compare the patient’s response with the appointment record, RXNT chart, identification, order, medication label, or specimen label.

Resolve Any Mismatch

Stop the workflow when information does not match. Review the chart, identification, demographics, and supporting documentation before proceeding.

Confirm the Correct Chart

Verify that the correct patient chart is open before documenting, placing orders, sending messages, or reviewing protected information.

Repeat Verification as Needed

Reconfirm identity before medications, injections, procedures, specimen collection, lab testing, referrals, or release of information.

When Identification Is Required

Check-In and Registration

Before updating demographics, scanning documents, collecting payment, or discussing account information.

Rooming and Vitals

Before rooming the patient or entering vitals, history, medication information, or the reason for the visit.

Medication Administration

Before administering any medication, vaccine, injection, treatment, or sample.

Specimen Collection

Before collecting, labeling, handling, or sending a specimen for testing.

Clinical Procedures

Before beginning a procedure, treatment, diagnostic test, or provider-assisted service.

Telephone and Portal Communication

Before discussing results, medication information, appointments, balances, or protected health information.

Orders and Referrals

Before creating or processing prescriptions, labs, imaging, referrals, or follow-up orders.

Medical Records

Before releasing records, completing forms, or sharing patient information with an authorized party.

Special Situations

Minors: Confirm the child’s full name and date of birth with the parent or legal guardian. Confirm the adult’s authority to consent when required.
Patients unable to respond: Use identification, available records, a parent, guardian, caregiver, or legally authorized representative to confirm identity.
Interpreter use: Use an appropriate interpreter when language or communication barriers prevent reliable verification.
Patients with similar names: Review additional identifiers carefully, including date of birth, address, phone number, and other available demographic information.
Incorrect demographic information: Do not overwrite information without verifying the correct patient and following the approved demographic update workflow.
Wrong chart opened: Stop immediately. Do not continue documenting. Notify the appropriate supervisor and follow the correction and incident-reporting process when information has been entered into the wrong chart.

Do and Do Not

Always Do

  • Ask the patient to state their information.
  • Use at least two identifiers.
  • Compare responses with the correct record.
  • Pause when information does not match.
  • Verify the correct chart before documenting.
  • Repeat verification before high-risk activities.
  • Protect the patient’s privacy while confirming identity.

Never Do

  • Ask, “Are you Jane Doe?” as the only verification.
  • Use room number or appointment time as an identifier.
  • Assume identity because you recognize the patient.
  • Continue when demographic information conflicts.
  • Document before confirming the correct chart.
  • Discuss private information where others can hear.
  • Ignore a possible duplicate or mismatched record.

Before Opening the Chart

Confirm the patient’s name and date of birth from the schedule or patient search results.

After Opening the Chart

Reconfirm the chart header and demographic information before entering documentation or orders.

Before Saving

Confirm that documentation, messages, orders, and attachments belong to the correct patient.

Duplicate Records

Do not create another patient record until an existing-record search has been completed carefully.

Demographic Changes

Verify the patient before changing names, addresses, phone numbers, insurance, or other demographic information.

Wrong-Chart Concern

Stop work, preserve the information, and escalate the concern rather than attempting to hide or independently delete the error.

Employee Competency

The employee should be able to demonstrate each of the following during onboarding or competency validation.

States the two primary patient identifiers.
Uses an open-ended identification request.
Verifies the correct RXNT chart.
Identifies when verification must be repeated.
Explains how to handle mismatched information.
Protects privacy during identification.
Recognizes a possible duplicate record.
Escalates a wrong-chart concern appropriately.
