Accurate patient demographic information is essential for patient safety, effective communication, successful insurance billing, regulatory compliance, and continuity of care. Every employee is responsible for ensuring patient information remains current and accurate at every encounter.

Purpose

Maintaining current demographic information ensures patients receive accurate care, appointment reminders, laboratory results, prescriptions, billing statements, referrals, and other healthcare communications without delay.

Patient demographic information should be verified during every patient interaction regardless of whether the patient reports changes. Small changes, such as an updated phone number or insurance plan, can significantly affect patient care and reimbursement.

Remember: Never assume information is still correct simply because the patient visited recently.

Responsibilities

Front Office

Verify demographics during scheduling, check-in, insurance verification, registration, and every patient visit.

Medical Assistants

Confirm patient identity and notify Front Office of any demographic discrepancies discovered during rooming.

Providers

Notify staff of incorrect patient information identified during the clinical encounter.

Billing Department

Verify demographic information whenever billing errors, returned mail, rejected claims, or insurance eligibility issues occur.

The following information should be reviewed with every patient during each visit.

Preferred pharmacy should be confirmed at every visit to reduce prescription delays.

Verify PCP information whenever insurance requirements or referral processes depend upon it.

Standard Workflow

Verify Patient Identity

Follow the Patient Identity Verification SOP by confirming two approved patient identifiers before making any demographic changes.

Open the Correct RXNT Record

Confirm the patient's name, date of birth, provider, and appointment before editing any demographic information.

Review Every Demographic Field

Ask the patient whether anything has changed since their previous visit. Review every section rather than only asking about insurance.

Update Information

Enter all demographic changes immediately into RXNT while the patient is present.

Verify Accuracy

Review the updated information one final time with the patient before saving.

Scan Supporting Documentation

If insurance cards or identification have changed, scan updated documents into RXNT immediately.

Best Practice: Every demographic update should be completed while the patient is physically present whenever possible.

Insurance Updates

Insurance information directly affects eligibility verification, prior authorizations, referrals, claim submission, and reimbursement. Changes should be updated immediately when identified.

Request Current Insurance Cards

Obtain the patient's current insurance card(s) at every annual visit, whenever coverage changes, or whenever eligibility cannot be confirmed electronically.

Verify Subscriber Information

Confirm subscriber name, relationship, member ID, group number, effective date, and payer information match the insurance card.

Scan Updated Cards

Scan both the front and back of each insurance card into RXNT before completing check-in.

Verify Eligibility

Run eligibility verification whenever available to identify inactive coverage, copay changes, deductibles, or referral requirements.

Important: Never assume an insurance plan is unchanged because the insurance card appears similar. Even minor plan changes may require different billing procedures.

Address, Phone Number & Email Changes

When a patient reports a new address, phone number, or email address, update the information immediately while confirming spelling and formatting with the patient.

Always verify:

• Primary Phone
• Secondary Phone
• Email Address

Legal Name Changes

Patients may request updates because of marriage, divorce, adoption, legal name change, or correction of previous registration errors.

Whenever appropriate, request supporting documentation before changing the patient's legal name in the medical record.
Never create a new patient chart because of a legal name change. Locate the existing patient record and update demographics accordingly.

Emergency Contact Updates

Verify Relationship

Confirm the emergency contact's relationship to the patient.

Confirm Phone Numbers

Verify all contact numbers remain current.

Review Annually

Emergency contacts should be reviewed at least annually or whenever the patient reports changes.

Document Updates

Update RXNT immediately after verification.

Special Situations

Minor Patients

Verify information with the parent or legal guardian while ensuring guardian information remains current.

Language Barriers

Use interpreter services when necessary to accurately verify demographic information.

Behavioral Health

Verify demographic information while maintaining patient privacy and confidentiality.

Returned Mail

Whenever mail is returned, verify the patient's address before the next visit or by telephone.

Patient Profile

Update demographics directly within the Patient Profile screen.

Insurance Module

Update insurance information and upload new insurance cards.

Documents

Upload updated identification and supporting documentation when applicable.

Clinical Notes

Document significant demographic corrections whenever clinically appropriate.
