🚶 Walk-In Patient Workflow
Purpose
Walk-in patients require prompt assessment to determine whether they can be safely accommodated without compromising scheduled patient care or delaying treatment for medical emergencies.
Walk-in patients are evaluated on a case-by-case basis based upon:
- Clinical urgency
- Provider availability
- Current clinic schedule
- Staffing resources
- Patient safety
Walk-In Decision Workflow
Department Responsibilities
Reception
- Welcome patient professionally.
- Determine reason for visit.
- Notify MA or management immediately.
- Do not promise same-day care.
- Remain calm and reassuring.
Medical Assistant / Management
- Perform brief clinical screening when indicated.
- Identify potential emergency symptoms.
- Communicate findings to provider.
- Provide objective information only.
- Await provider approval before registration.
Provider
- Determine if patient can safely be worked into schedule.
- Consider patient acuity.
- Consider clinic capacity.
- Provide final disposition.
Patients Requiring Immediate Emergency Referral
Do NOT Attempt to Work Into the Schedule
Patients presenting with any of the following should immediately be directed to call 911 or proceed to the nearest Emergency Department. Staff should notify clinic leadership immediately.
- Chest pain
- Stroke symptoms
- Difficulty breathing or respiratory distress
- Loss of consciousness
- Severe uncontrolled bleeding
- Major traumatic injuries
- Active seizures
- Suicidal or homicidal ideation requiring emergency intervention
Walk-In Conditions Generally Appropriate for Evaluation
Provider Availability Required
The following concerns may be considered for same-day evaluation based on provider availability and clinic capacity.
- Cold or flu symptoms
- Sore throat
- Ear pain
- Urinary symptoms
- Minor injuries
- Medication concerns
- Skin rashes
- Minor allergic reactions without airway involvement
- Simple wound evaluations
- Blood pressure concerns
Approved Walk-In Workflow
Once the provider or management approves the walk-in patient, staff should begin the standard registration and intake process. Every approved walk-in should receive the same high-quality experience as a scheduled patient while minimizing delays to existing appointments.
Registration Requirements
Following approval, Reception should complete the standard patient registration workflow before the patient is roomed.
- Verify patient identity using two identifiers.
- Confirm demographic information.
- Scan identification and insurance cards.
- Verify insurance eligibility when applicable.
- Update emergency contact information.
- Ensure all required consent forms have been completed.
- Collect any required copay or self-pay amount.
Registration should not begin until provider or management approval has been obtained unless the patient is experiencing a medical emergency requiring immediate intervention.
Medical Assistant Workflow
Room Preparation
- Escort patient to exam room.
- Verify two patient identifiers.
- Document chief complaint.
- Obtain vital signs.
- Review medication list.
- Review allergies.
- Update pharmacy information if needed.
Clinical Documentation
- Document all intake information in RXNT.
- Notify provider that patient is ready.
- Report abnormal vital signs immediately.
- Document any additional patient concerns.
- Remain available for provider assistance.
Provider Evaluation
- Complete assessment.
- Order medications, labs, or imaging as appropriate.
- Document encounter.
- Provide patient education.
- Complete visit disposition.
When a Walk-In Cannot Be Accommodated
When the provider determines that the clinic cannot safely accommodate the patient, staff should communicate the decision professionally while assisting the patient with the next appropriate step.
Schedule Next Available Appointment
- Offer the earliest available appointment.
- Add patient to cancellation list if appropriate.
- Review symptoms that would require urgent medical attention.
Refer to Urgent Care
- Patient requires same-day evaluation.
- Clinic schedule cannot safely accommodate visit.
- Condition is not life-threatening.
Refer to Emergency Department
- Life-threatening symptoms.
- Potential stroke or heart attack.
- Respiratory distress.
- Major trauma.
- Any condition requiring emergency stabilization.
Documentation Requirements
- Document the patient's chief complaint.
- Document provider approval or denial.
- Record patient disposition.
- Complete all required RXNT documentation.
- Document referrals when applicable.
- Document patient education provided.
- Complete encounter documentation according to clinic policy.
Customer Service Standards
Every Walk-In Patient Should Leave Feeling...
- Welcomed and respected.
- Heard without feeling rushed.
- Confident they received appropriate guidance.
- Informed of the next steps in their care.
- Treated with empathy regardless of whether they were seen that day.
Being unable to accommodate a walk-in should never result in a negative patient experience. Staff should remain courteous, explain the reason for the decision, and assist the patient in obtaining the appropriate level of care.
