🌬️ Nebulizer Treatment Administration

To establish a standardized process for administering nebulized medications safely while ensuring accurate medication delivery, respiratory assessment, infection prevention, patient monitoring, and appropriate documentation.

Purpose

Nebulizer treatments are administered to deliver inhaled medications directly to the lungs to relieve bronchospasm, wheezing, and other respiratory symptoms. Clinical staff are responsible for ensuring the correct medication is administered safely while monitoring the patient's respiratory status before, during, and after treatment.

Policy

Nebulizer treatments may only be administered with a valid provider order. Medical Assistants must remain within their delegated scope of practice and immediately report any unexpected patient response to the provider.

Pre-Treatment Assessment

Before administering any nebulized medication, complete and document an appropriate respiratory assessment.

  • Verify the patient using two identifiers.
  • Review the provider order.
  • Confirm medication allergies.
  • Review previous reactions to nebulized medications.
  • Obtain baseline vital signs.
  • Measure oxygen saturation (SpO₂).
  • Assess respiratory rate and work of breathing.
  • Auscultate lung sounds if within clinic protocol.
  • Document the patient's respiratory symptoms.

Baseline Assessment

Document respiratory effort, presence of wheezing, coughing, accessory muscle use, oxygen saturation, and the patient's subjective breathing difficulty before treatment begins.

Equipment

  • Nebulizer compressor.
  • Nebulizer medication cup.
  • Tubing.
  • Mouthpiece or face mask.
  • Pulse oximeter.
  • Stethoscope.

Medication & Safety

  • Provider-ordered medication.
  • Normal saline if ordered.
  • Gloves.
  • Alcohol prep pads.
  • Biohazard waste container.
  • Emergency equipment readily available.

Do NOT Begin Treatment If:

  • The provider order is incomplete.
  • The medication cannot be verified.
  • The medication is expired.
  • The patient has a documented allergy to the medication.
  • The nebulizer equipment is malfunctioning.
  • The patient demonstrates signs of respiratory distress requiring immediate emergency intervention.
  • You have concerns regarding the patient's stability.

Medication Preparation & Administration

Prepare the nebulizer medication immediately before administration using the provider's order and the manufacturer's instructions. Always follow the Medication Rights prior to administration.

  • Perform hand hygiene before handling medication.
  • Verify the patient's identity using two approved identifiers.
  • Confirm the provider order.
  • Verify the correct medication, dose, route, and expiration date.
  • Inspect medication for discoloration or damage.
  • Assemble the nebulizer according to manufacturer recommendations.
  • Place the medication into the nebulizer cup without contaminating sterile components.
  • Attach tubing and connect to the compressor.

Verify the right patient, right medication, right dose, right route, right time, right documentation, right reason, right response, and the patient's right to refuse treatment.

Administering the Nebulizer Treatment

Ensure the patient is positioned to maximize lung expansion before beginning treatment.

4️⃣ Coach Slow Deep Breathing
5️⃣ Continue Until Medication Is Gone
6️⃣ Disconnect Equipment

Patient Coaching

  • Breathe slowly through the mouth.
  • Take deep breaths when able.
  • Hold each breath briefly before exhaling if tolerated.
  • Avoid talking during treatment.
  • Notify staff immediately if symptoms worsen.

Patient Monitoring During Treatment

Remain available to observe the patient throughout treatment and reassess if symptoms change.

  • Observe respiratory effort.
  • Monitor oxygen saturation if indicated.
  • Assess work of breathing.
  • Monitor for improvement in wheezing.
  • Observe for dizziness or anxiety.
  • Monitor heart rate if ordered or clinically indicated.
  • Ensure medication is being aerosolized appropriately.

Stop Treatment Immediately & Notify the Provider If:

  • Respiratory distress worsens.
  • Chest pain develops.
  • Oxygen saturation decreases significantly.
  • The patient becomes cyanotic.
  • Severe dizziness or syncope occurs.
  • Signs of allergic reaction develop.
  • The patient cannot tolerate the treatment.

Post-Treatment Reassessment

Once the nebulizer treatment has been completed, reassess the patient's respiratory status and document the clinical response.

  • Repeat vital signs when indicated.
  • Repeat pulse oximetry (SpO₂).
  • Assess respiratory effort.
  • Evaluate lung sounds if within clinic protocol.
  • Assess patient-reported symptom improvement.
  • Report any concerning findings to the provider.

  • Reduced wheezing.
  • Improved air movement.
  • Decreased shortness of breath.
  • Improved oxygen saturation when appropriate.
  • Patient reports easier breathing.

Cleaning & Infection Prevention

Proper cleaning of reusable equipment reduces the risk of cross-contamination and helps maintain equipment performance.

  • Discard all single-use components after treatment.
  • Clean reusable equipment according to manufacturer recommendations.
  • Disinfect reusable components between patients.
  • Allow equipment to dry completely before storage.
  • Store supplies in a clean, dry location.
  • Perform hand hygiene after equipment handling.

  • Date and time of treatment.
  • Medication administered.
  • Dose administered.
  • Route of administration.
  • Provider order verified.
  • Pre-treatment assessment findings.
