Exam Room Cleaning & Turnover

Every patient deserves a clean, safe, and fully prepared exam room. Proper room turnover reduces the risk of cross-contamination, improves provider workflow, supports infection prevention, and ensures the next patient is welcomed into a professional clinical environment.

Primary Users Medical Assistants & Clinical Staff
Frequency After Every Patient Encounter

Purpose

Consistent room turnover protects patients and staff by maintaining a clean, organized, and safe clinical environment between every patient encounter.

Immediately after a patient leaves the exam room, staff should clean, disinfect, restock, and inspect the room before preparing it for the next patient.

Important: Room turnover should be completed immediately after patient departure whenever possible. Rooms should never remain dirty or partially cleaned between visits.

Responsibilities

Medical Assistants

Clean and disinfect the exam room, restock supplies, inspect equipment, and prepare the room for the next patient.

Providers

Notify clinical staff of any unusual contamination, equipment concerns, or additional cleaning requirements.

Leadership

Monitor cleaning compliance, supply availability, and infection prevention standards.

All Clinical Staff

Maintain a clean clinical environment and report safety concerns immediately.

Standard Room Turnover Workflow

1

Perform Hand Hygiene

Complete hand hygiene and apply appropriate PPE before beginning room cleaning.

2

Remove Disposable Materials

Discard exam table paper, disposable supplies, and waste using the appropriate waste containers.

3

Clean & Disinfect High-Touch Surfaces

Disinfect the exam table, countertops, chairs, blood pressure equipment, light switches, door handles, keyboards, and other frequently touched surfaces using approved disinfectants.

4

Inspect Equipment

Verify all reusable equipment has been cleaned and remains functional before returning it to service.

5

Replace Exam Table Paper

Apply fresh exam table paper and return furniture to its standard room layout.

6

Restock the Room

Replace gloves, gauze, alcohol pads, specimen containers, tongue depressors, and other commonly used supplies before the next patient arrives.

Best Practice: Complete room turnover immediately after every patient while details of the visit are still fresh and before beginning another task.

Biohazard & Sharps Management

Proper disposal of regulated medical waste protects patients, staff, and the environment while ensuring compliance with OSHA, CDC, and clinic infection prevention standards.

Sharps Containers

Dispose of needles, lancets, scalpel blades, and other sharps immediately after use. Never recap needles unless an approved safety device requires it.

Biohazard Waste

Dispose of contaminated materials in designated biohazard containers according to clinic policy.

Regular Waste

Non-contaminated materials should be discarded in standard trash receptacles.

Overflowing Containers

Notify leadership immediately if sharps or biohazard containers approach their fill limits. Replace containers before they become overfilled.

Cleaning Standards

All cleaning should be completed using approved disinfectants and according to manufacturer contact times. Surfaces must remain visibly wet for the required disinfection period before drying.

High-touch surfaces include:

• Exam table
• Countertops
• Chairs and stools
• Blood pressure cuffs
• Pulse oximeters
• Thermometers
• Door handles
• Light switches
• Computer keyboards and mice (per manufacturer recommendations)

Special Cleaning Situations

Blood or Body Fluid Exposure

Follow the Bloodborne Pathogens Exposure SOP and use appropriate PPE before cleaning contaminated areas.

Isolation Precautions

Follow additional cleaning procedures required for patients on transmission- based precautions.

Equipment Contamination

Remove contaminated equipment from service until it has been properly cleaned and disinfected.

Supply Contamination

Discard single-use contaminated supplies immediately. Never return contaminated items to clean storage.

Room Readiness Verification

Before the next patient is escorted into the room, complete one final inspection.

7

Inspect the Room

Verify the room is clean, organized, and free of used supplies or equipment.

8

Verify Supplies

Confirm commonly used supplies have been replenished and specialty items are available when needed.

9

Prepare for the Next Patient

Ensure the room meets the standards outlined in the Exam Room Preparation SOP before bringing in the next patient.

Best Practice: Every patient should enter a room that looks exactly as if they are the first patient of the day.

Common Turnover Errors

Incomplete Disinfection

Missing high-touch surfaces increases the risk of cross-contamination.

Failure to Restock

Running out of supplies during patient care interrupts workflow and delays treatment.

Improper Waste Disposal

Incorrect disposal of sharps or biohazard waste creates unnecessary safety risks.

Skipping Final Inspection

Always perform one last walkthrough before considering the room ready for the next patient.
